APPLICATION GUIDELINES

We request that all students submitting work must complete a Release Form prior to beginning class. This form permits Future Generations University to display your work in official print and web publications related to the mission of the University. Please note that Future Generations will never sell your information, examples of your coursework, or pictures of you to any other entity.

If you agree to each release, please check “I agree.” If you would rather we do not display your work or likeness, please check “I disagree.”

A. USE OF STUDENT WORK / ASSIGNMENTS

I hereby authorize Future Generations University to scan, reproduce, transmit and/or otherwise display any or all my work submitted as assignments on a royalty-free basis.

• I warrant that any work submitted for assignments is original and entirely owned by myself.

• I recognize that any display of my work may be accompanied by a brief description of the work that I have submitted (such description may be edited by the University in the event that space is limited) .

• I recognize that Future Generations University reserve the right to edit, crop or layer images for layout purposes in University publications and website.

B. PHOTO RELEASE

I hereby grant permission to Future Generations University to use photographs and/or video of me in publications, news releases, online, and in other communications related to the mission of Future Generations University.

C. Agreement to Release and Hold Harmless

I agree to release and hold harmless Future Generations University and its Trustees, employees, agents and/or representatives from any and all direct, indirect, special or consequential damages or costs, legal or otherwise, which I may incur as a result of the display of my work by Future Generations University. In submitting this release, I acknowledge and represent that I have read the foregoing and understand it; no representations apart from the foregoing written agreement have been made; and I execute this Release/Agreement for adequate consideration fully intending to be bound by the same
